Abstract
저자들은 하악의 골동통을 주소로 내원한 46세 여자환자에서 임상 ·방사선학적 및 조직
병리학적 소견을 통해 증례를 관찰하고 다음의 결과를 얻었다.
1. 임상적으로 양측 하악체 부위의 간헐적인 동통과 하순의 지각마비, 쇄골골절로 인한 동
통과 압통이 관찰되었다.
2. 혈청단백질 전기영동에서 M-Peak가 나타났으며, 과칼슘증과 적혈구 침강속도가 증가
하였으나, 뇨중 Bence-Jones단백은 검출되지 않았다.
3.방사선학적으로 병소에 인접한 치근의 흡수와 함께 양측 하악체부, 상악골, 흥골, 쇄골
및 사지 등의 부위에서 다발성의 방사선투과성 병소들이 산재되어 관찰되었고, 골 스캔상에
서 병소부위에 방사선 동위원소의 집적이 증가되어 관찰되었다.
4. 조직병 리학적으로 형질세포가 풍부한 과세포성 골수의 소견을 보였고, 골수종 세포의
증식과 거핵구의 숫적 증가가 관찰되었다.

Multiple myeloma is a malignant plasma cell tumor that is thought to originate
proliferation of a single clone of abnormal plasma cell resulting production of a whole
monoclonal paraprotein. The authors experienced a case of multiple myeloma with severe
mandibular osteolytic lesions in 46-year-old female. As a result of careful analysis of
clinical, radiological, histopathological features, and laboratory findings, we diagnosed it
as multiple myeloma, and the following results were obtained:
1. Main clinical symptoms were intermittent dull pain on the mandibular body area,
abnormal sensation of lip and pain due to the fracture on the right clavicle.
2. Laboratory findings revealed M-spike, reversed serum albumin-globulin ratio,
markedly elevated ESR and hypercalcemia.
3. Radiographically, multiple osteolytic punched-out radiolucencies were evident on the
skull, zygoma, jaw bones, ribs, clavicle and upper extremities. Enlarged liver and
incirased uptakes on the lesional sites in RN scan were also observed.
4. Histopathologically, markedly hypercellular marrow with sheets of plasmoblasts and
megakaryocytes were also observed.
